Important Forms

Section 419(c) of Public Law 108-203, the Social Security Protection Act of 2004, requires State and local government employers to disclose the effect of the Windfall Elimination Provision and the Government Pension Offset to employees hired on or after January 1, 2005, in jobs not covered by Social Security. This particular law requires newly hired public employees to sign a statement that they are aware of a possible reduction in their future Social Security benefit entitlement.

Direct Deposit Paystubs Can be Emailed

Auditor Mike Kovack has announced that employees on direct deposit now have the option to have their paystubs emailed to their personal email account. The paystub will be in PDF format and look exactly like the hardcopy currently distributed. Employees can either print a copy or save it to a file on their home computer.

Paystubs will be sent from the email address: MedinaCountyDD@Medinaco.org.

Employees electing this option must first complete a form and return it to the person who processes their department’s payroll. Employees must use a non-Medina County government email account, e.g. Hotmail, Gmail, Yahoo, to receive their paystub via email.
