Help Shape the Future of Community Health

Residents across Medina, Portage, Stark, Summit and Wayne counties are invited to share their perspectives on the health and wellness needs of their communities by participating in a brief survey from Trailhead Community Health Foundation.

Trailhead Community Health Foundation was created to invest in programs and initiatives that improve the health and well-being of residents throughout the five-county region. As the foundation develops its first strategic plan, community feedback will play a critical role in determining where future investments are made.

The survey takes less than five minutes to complete and asks participants about topics such as access to healthcare, community resources, barriers to receiving care, and opportunities to improve health and wellness. Residents will also have the opportunity to share ideas for innovative solutions and provide additional comments about the needs they see in their communities.

Responses will help the foundation better understand the challenges residents face and identify priorities that can have the greatest impact for years to come.

The survey is open through Tuesday, July 31, and all residents of Medina, Portage, Stark, Summit and Wayne counties are encouraged to participate.
